In the scenario my client wants to make a claim on this policy, how do I go about this?

Your client can fill out these claim forms for Total Permanent Disability claim and Accidental Death claim.  The list of required supporting documents are detailed in the form.  Upon completion of the form, they may send to TMLS via the following address:
